Brooke Place
Target Population: Girls aged 12-18 are referred to gender-specific
treatment at Brooke Place from a variety of sources:
- WVDHHR
- Juvenile Probation
- MDTs
- Guardians ad litem
- Community mental health agencies
- Therapists
- Families

All referrals must eventually be put through the Automated Placement
Referral (APR) by the girl’s WVDHHR caseworker.
Referrals are reviewed and responded to by the Brooke Place Director.
Sometimes additional information is requested. In general, the
information needed to make a decision about placement includes:
- CAPS (Comprehensive Assessment and Planning System)
- Psychological evaluation
- Psychiatric treatment
- Placement history
- Discharge summaries
- Family involvement/motivation
- Medical needs
- Current medication
- Education status
- Assessment for self-administration of medication
Once there is mutual agreement that Brooke Place is the placement of
choice, an intake date is set. While the caseworker is responsible for
signing the intake documents, it is desirable for at least one adult
family member to be present at admission (unless contraindicated). This
is a first opportunity for an introduction to Brooke Place staff, to
make a tour of the facility, and receive a handbook with the kind of
information parents often want to know.
Brooke Place will make every effort to maintain the connection between a
girl and her family as long as this contact is not contraindicated.
This means families may visit at any time with just a courtesy phone
call to let staff know they are coming. Family members can join us for
dinner, help with homework, and participate in or observe the routine of
daily living. Family members cannot regularly take the resident off
grounds or remove her from individual or group counseling sessions.
Joining the residents on activities is possible but must be arranged
with the Brooke Place Director. Accommodations are available in the
private family room for parent(s) who wish to visit overnight.
If it is decided that placement at the group home is appropriate, contacts
are made with the interested parties-the caseworker, probation, other
agency, family, etc. A date is set for intake.
Brooke Place has success with girls fitting these general
guidelines:
- She has experienced at least one traumatic life event.
- Her MDT is clear about the criteria for success and is respectful of
the treatment process.
- She is able to identify some of the concerns that have led to
placement.
- She has an immediate or extended family who will be involved in
treatment beyond availability for home visits.
- She is truant and/or has had little success in school.
- She has experimented with drugs and/or alcohol, but her use is in
the mild to moderate range.
- Her permanency plan is for a return home, foster care, or
independent living.
- Her safety and the safety of other residents can be maintained.
Brooke Place does not maintain a waiting list for residency. Referrals are considered in conjunction with openings available and on a high risk-high need basis. If you are a caseworker and wish to view our current openings, go to
